Final Forms for the 2026-2027 school year will open on Friday, May 1. This means that sports physicals can be scheduled and completed for the upcoming school year. Please remember that physicals are required for all athletes in grades 7 – 12 who will be participating in a school sponsored sport. Yearly physicals must be completed prior to the first day of practice. Once a student is added to a sport in Final Forms, all of the required athletic forms will appear. This will also allow you to print the blank OHSAA physical form that needs to be taken to the doctor to be cleared to play. This document is 5 pages long so please make sure you have all of the pages printed before going to get your physical. Pages 1 - 3 include the health history questions that you need to answer and pages 4 - 5 include the actual physical form that will be completed at your physical. After the doctor signs off, then the form can either be turned into the school office or uploaded to Final Forms. Please note that not all school forms will appear right away. Some forms will not be viewable until they have been board approved.

Free Physicals Offered by Grand Lake Health
This year we are once again teaming up with New Bremen and Grand Lake Health System to offer a day of free physicals on Monday, June 15. Attached is the flyer with all of the information. Please make sure that your final forms documents are completed prior to coming to this free clinic and bring the appropriate paperwork with you. Please also note the times for the different grade levels, which are listed on the flyer.
